Case Study · ACL & Meniscal Reconstruction

How H.A.A. rebuilt a fully torn ACL and got back to MMA training in 16 weeks.

At 23, MMA fighter H.A.A. tore his right ACL, both menisci and the cartilage over his medial femoral condyle in a single injury. OSSO fixed all four in one arthroscopic procedure, repairing the menisci rather than removing them. Sixteen weeks later: pain 1/10, full range of motion, back in supervised training.

Patient Profile

H.A.A., 23 · MMA Fighter

Three months earlier, H.A.A. injured his right knee during play. It swelled, gave way, and wouldn't bend past 90°. No running, no pivoting, no fighting. MRI: complete ACL rupture, both menisci torn, cartilage lesion on the medial femoral condyle. Referred to OSSO by Ashwin Hoon.

Diagnosis

Right knee - ACL, both menisci, cartilage

Prior Recommendation

Surgery recommended

OSSO Protocol

ACL reconstruction, meniscal repair, BMAC cartilage, rehab

Duration & Outcome

16 weeks · Returned to supervised MMA training

The Full Journey

From a knee that gave way to agility drills H.A.A.'s 16 weeks with OSSO.

A complete look at how H.A.A. came to OSSO, what the surgery and the rehabilitation actually involved, and where he is now.

Chapter One

How H.A.A. found OSSO

The injury happened during play and never settled. Three months on, the knee was still swollen, still giving way, and moving through barely 10° to 90°. His quadriceps had shut down, walking was difficult, and running, jumping and pivoting were out of the question which, for a fighter, is the whole sport. At 23, with a career built on that knee, waiting any longer wasn't an option. The MRI that followed didn't stop at the ligament, it showed both menisci torn and a cartilage lesion on the medial femoral condyle as well, far beyond what the injury first looked like. He had already been told he needed surgery. What he had not been told was what kind, or what would happen to the rest of the joint. Ashwin Hoon sent him to OSSO.

Treatment Timeline

The 16-week protocol, phase by phase.

Every OSSO plan is built around the individual knee here is exactly how H.A.A.'s unfolded.

Week 0–2

Protect & Move

Pain and swelling control, protected weight-bearing, and early range-of-motion work.

Week 3–6

Motion & Activation

Progressive range of motion, gait correction and targeted muscle activation.

Week 7–12

Strength & Control

Quadriceps strengthening, proprioception and balance training.

Week 13–16

Agility & Sport Prep

Functional strengthening, agility drills, and the start of sport-specific rehabilitation.

Week 16 →

Return to Sport

The advanced return-to-sport programme continues.

The Measured Difference

Before treatment vs. Week 16.

Clinical measurements taken at intake and at the close of the 16-week protocol. Recovery after ACL reconstruction with meniscal and cartilage repair varies from patient to patient; clearance for unrestricted competition typically follows at 9–12 months.

Treatments Used in This Case

Repair first, replace last.

Some injuries do need the operating theatre. What changes is what happens once you are in it - every structure that could be preserved in this knee was preserved rather than removed.

ACL Reconstruction

Arthroscopic reconstruction of the ruptured ligament using the patient's own tissue, restoring the knee's primary stabiliser.

Meniscal Repair

The medial and lateral meniscal tears were repaired rather than trimmed away, preserving native tissue and protecting the joint long term.

Cartilage Repair

The cartilage lesion on the medial femoral condyle treated biologically using Bone Marrow Aspirate Concentrate.

Rehabilitation

Supervised physiotherapy 3–5 sessions a week, tapering with progress, plus daily home exercise, cycling and balance work.
